To obtain a Consent to Join and Release Form, please enter your 8-digit Claim Number and your 6-digit Control Number. Your Claim Number and Control Number were included on page 1, directly under your contact information, on the Notice you received via mail and/or e-mail. If you lost your Claim Number or Control Number or if you did not receive the Consent to Join and Release Form in the mail or e-mail and feel you are a class member, please call the settlement administrator at (866) 602-2260 or complete the contact us form.
